Serum B12 and folate level in alcoholics and nonalcoholics: Correspondence

We read the publication on "A cross-sectional study of serum B12 and folate level in alcoholics and nonalcoholics" with a great interest. Determining hematological changes in chronic alcoholics and facilitating therapy and adequate psychiatric counseling, according to Myilsamy et al., will reduce the risk of morbidity and mortality among alcoholics, as well as complications such as liver cirrhosis, renal and heart disease, cerebellar degeneration, pancreatitis, neural disorders, and others. We both agree that hematological parameters or serum folate/Vitamin B12 levels in alcoholics may vary. However, it is critical to understand the confounding variables. In areas with a high incidence of hemoglobinopathy, the hematological parameter, serum folate, and Vitamin B12 may be confounded. For example, independent of alcohol consumption, thalassemic patients often have lower serum levels of folate and Vitamin B12 than healthy controls.
